Why do I need a well visit before my child can get his/her immunizations?
Well visits are an important part of your child’s overall health care. During these visits, the doctor/nurse practitioner will do a comprehensive review of your child’s growth and development. We will also provide safety and anticipatory guidance, which includes an in-depth review of the vaccine record. Sick visits generally do not allow enough time for this. During a physical, your doctor may pick up on many diseases earlier, generally requiring less intervention. Examples include tumors, scoliosis, cancer, hearing loss, vision problems and more.
My statement says “insurance requests additional information.” What does that mean?
Your insurance company is waiting for you to contact them to answer a few questions, generally asking if you have another insurance plan. They probably sent you a questionnaire that they are waiting to receive back. A phone call to your insurance company will usually work, too. After they hear from you, they will process the claim. If you do not contact them, the bill will become your responsibility.
How do I get a copy of my immunization record?
Call our appointment line or NurseLine, press prompt 4 from the phone menu, leave the information requested and we will have your request completed within 48 business hours. Please note that there is a fee for stat requests. Also, be sure to make a copy for your records because there is a fee for additional copies.
How do I get a copy of my medical records?
Complete a medical record request form (available here) and either bring it to the office or fax it to us. We have 30 days to copy these and get them to you, but generally have them ready within two weeks. If you need them sooner, please let us know. Depending on how quickly you need them, a stat fee may apply.

How do I make a payment?
You can view your statement and pay online through our practice portal or you can mail in your payment to 10011 S. Yale Ave., Suite 200, Tulsa, OK 74137.
How can I get a medication refilled?
Call your pharmacy and they will contact us with the refill request. The best time to call them is one week before you run out of the medication. This allows for plenty of communication time between our office and the pharmacy. If you need an ADD medication refilled, you will have to pick up the written prescription at the office. This is required by law. Please allow 48 hours before picking up the prescription from our office.
Why do I have to pay a co-payment for a follow up appointment?
The co-payment is set and applied by your insurance company; we have no control as to the amount of the co-payment or what visits it applies to. If we do not collect the co-payment that your insurance company requires, we are then in violation of our contract with them.
