For Newborns: Remember to add your newborn to your insurance within the first 30 days after birth to avoid lapse in coverage. We will need your baby’s insurance card by the two-month well-child visit.

For Everyone:Please remember to bring your child’s insurance card with you to appointments. It is essential that we have correct insurance information so you can avoid unnecessary out-of-pocket expenses.
